Are you familiar with advance care planning? As more people care for family members living with chronic illness at home, questions and concerns may arise about what will happen if their health declines.

Many Canadians have not yet heard of advance care planning (ACP). It is a process of reflecting on and communicating your values, wishes and preferences for future health and personal care with your family, those close to you and your health care team.

This interactive session will explore what advance care planning involves, how to begin the conversation and how to approach difficult decisions. There will also be an opportunity to ask questions.

More family members are caring for people with chronic illnesses at home. The experience can be both rewarding and challenging, often giving rise to complex and sometimes conflicting emotions. This interactive session will explore the changing roles and expectations that come with becoming a family caregiver, as well as your readiness to take on this responsibility. We will consider how caregiving can shape your sense of identity and your relationship with the person you care for—an important aspect of the experience that is rarely examined or discussed. You will also have an opportunity to reflect on and share your own caregiving experience.
